Step-by-Step PFM Crown Preparation
1. Tooth Reduction
Occlusal Reduction (1.5 - 2 mm):
Use a round-end tapered diamond bur to create functional and non-functional cusp clearance.
Maintain anatomical contours.
Axial Reduction (1.2 - 1.5 mm):
Reduce the buccal and lingual surfaces while keeping a uniform taper (6-10°).
Buccal reduction should be 1.5 mm for esthetic porcelain coverage.
Lingual reduction can be 1.0 - 1.2 mm if metal occlusion is planned.
2. Margin Design
Buccal Margin:
Heavy chamfer or Shoulder (1.2 - 1.5 mm) to support porcelain.
Lingual Margin:
Light chamfer (0.5 - 1 mm) if metal is on the lingual surface.
Interproximal Clearance:
Ensure at least 1 mm space to accommodate the crown.
3. Finishing & Evaluation

재보철치료과정에서의 단상
1. subgingival margin - 치은연하부분은 레진코어, 프렙, 인상채득, 세팅등 모든과정이 어려워집니다. 게다가 프렙량도 많아질수 밖에 없습니다. 파절, 우식등으로 이미 연하로 된 부분을 제외하고는 가급적 연상으로 진행을 하고, 마지막에 프렙할때 보이는 부분만 연하로 내리는게 최선인거 같습니다. 이 케이스에서도 원심부분이 우식으로 깊어서 그 부분이 우식제거, 레진코어할때도 까다로웠고, 프렙은 어렵진 않았지만 삭제량이(근심에 비해서) 많아졌습니다.
2. 하악대구치 프렙시 원심설측은 항상 어렵습니다. 보이지도 않고, 혀도 있고...그 부분을 잘하려고 끙끙대기보다는 일단 잘보이는 부분들을 해놓고, 원심설측은 마지막에 집중에서 끝내면 훨씬 수월한거 같습니다. 다른 부분을 어느 정도 해놓으면 원심설측 깎을 부분이 별로 많지 않은거 같습니다.
3. 경험이 쌓이면서 여러가지 요령과 꼼수(?)가 늘어가는거 같습니다. 하지만 이런 요령들도 결국 기본적인 원칙위에 있을때 빛을 발하는거 같습니다.
